Eligibility Categories
Only organizations operating as established MD STARnet sites under RFA DD-06-002 or DD-08-002 are eligible to submit an application under this FOA. These eligible sites include: 1. University of Arizona, College of Medicine, 2. Colorado Department of Public Health and Environment, 3. University of Iowa, College of Medicine, 4. New York State Department of Health, and 5. Hawaii Department of Health. Existing MD STARnet sites have an established population-based Duchenne/Becker muscular dystrophy surveillance system utilizing a unique protocol and methods to collect longitudinal data on individuals with DBMD, and provide the basis for this expansion demonstration effort. Limiting eligibility to these sites enables the continuation of longitudinal data collection on over 700 individuals with DBMD and for efficient expansion of data collection to other muscular dystrophies.

Grant Description
This funding opportunity announcement (FOA) has two components (A and B). CDC solicits UO1 grant applications to support Component A Core - to characterize the population of individuals who have muscular dystrophies through modification and expansion of MD STARnet. The ultimate goal is to improve access to care and the health of individuals with muscular dystrophies (MDs). All eligible applicants for component A may also apply for Component B – Enhanced Activities (1. Data Coordinating Center (DCC); 2. Abstractor training and quality assurance/control activities; 3. National coordinating center; and 4. Georgia healthcare transitions interview) to support ongoing MD STARnet activities.
