Grant Description
This Program Announcement provides an opportunity for states to demonstrate innovative approaches to improving the delivery of services and supports at the community-level to people with ADRD and/or their family caregivers. Under this Announcement, AoA is particularly interested in community-level projects that help individuals with ADRD who are not eligible for Medicaid but who are at imminent risk of nursing home placement and spend-down to Medicaid to remain at home and in the community. AoA is also interested in projects that are targeted at people with ADRD who are in the early stages of the disease. The Administration on Aging (AoA) plans to fund approximately 15 cooperative agreements under this Announcement, with an average federal funding level of approximately $300,000 per award for an 18 month project period. Applications will be accepted from all agencies of State Government, which includes but is not limited to State Units on Aging (SUAs). Applicants that are not SUAs must involve the SUA as a major partner in the planning, development and implementation of their proposed project. AoA is simultaneously issuing two (2) ADDGS Program Announcements that provide states with an opportunity to secure FY2008 grant funds to improve the delivery of services and supports to people with dementia and their caregivers: -- Evidence-Based Intervention Grants to Better Serve People with Alzheimer's Disease and Related Disorders: HHS-2008-AoA-AI-0812. -- Innovation Grants to Better Serve People with Alzheimer's Disease and Related Disorders: HHS-2008-AoA-AI-0813. States can apply for grants under either one or under both of these ADDGS Announcements.
