Justification for Less than Maximum Competition: Ministries of health are uniquely positioned to carry out the activities outlined in this funding opportunity announcement. Ministries of health serve as the lead for chronic disease programs in every island jurisdiction. They are uniquely positioned to provide leadership for chronic disease prevention and health promotion efforts across the island through a strong network of partnerships that are in place to expand access, availability and use of evidence-based interventions. Funding provided to all island jurisdictions through this cooperative agreement will support the implementation of activities that are outlined in the Non-Communicable Disease Plan developed by Ministries of health and submitted to CDC in FY 2013. Ministries of health are the only entities that have oversight over the implementation of these CDC approved Non-Communicable Disease Plan. Having established relationships within and outside of government allows for leveraging both time and staff resources to be able to immediately focus on the work of expanding access to interventions. Also, Ministries of health have access to existing personnel who are able to provide technical assistance based on their area of expertise (e.g., policy and systems development, communications, epidemiology, partnership development, and evaluation) which conserves valuable resources.

Grant Description
The primary purpose of this funding is to support the implementation of cross-cutting, unified approaches to promote health, prevent and control tobacco use, diabetes, heart disease, promotion of health pregnancy and infancy through primary and secondary prevention of NCDs in the US Affiliated Pacific Islands, Virgin Islands, and Puerto Rico. This FOA attempts to coordinate activities that address common risk factors, as well as cross-cutting epidemiology and surveillance, environmental approaches, health systems changes and clinical-community linkages (e.g. surveillance, screening, evaluation, data systems) and reduces the administrative and reporting requirements.
