These assessments further clarified the conditions contributing to the disproportionate burden of heart disease and stroke, and related conditions in the Delta as compared to the nation. During Phase Two, CDC laid the groundwork for the Mississippi Delta Health Collaborative through a planning phase, which included the development of a Strategic Alliance Advisory group and the development of a strategic plan. As part of this phase, community organizations were brought on board to implement evidence-based interventions in healthcare, community, faith-based, worksite, and schools.
