Grant Description
The purpose of this program is to provide support through CDC cooperative agreements for education and outreach activities aimed at preventing secondary conditions in people with bleeding disorders by improving their access to public health programs and implementing effective health promotion and wellness programs. Applicants can apply for one or more of the following modules: (A) Provision of a national clearinghouse and information resource center and evaluation of existing materials; (B) Promoting health and wellness for newly diagnosed families; (C) Raising awareness and providing education and outreach to women with bleeding disorders; (D) Promoting a successful transition of young adults to adulthood through advocacy and self-management skills; (E) Promoting increased physical activity and healthy weight; and (F) Promoting wellness among aging adults with bleeding disorders.
