Applications may be submitted by public and private non-profit and for profit organizations, and by governments and their agencies such as: public non-profit organizations; private non-profit organizations; small, minority, women owned businesses; universities; colleges; hospitals; community-based organizations and faith-based organizations. All applicants must possess the documented experience and ability to conduct public health programs/ projects. SPECIAL ELIGIBILITY CRITERIA: Licensing/Credential/Permits Cost Sharing or Matching: Cost sharing or matching funds are not required for this program. Maintenance of Effort: Maintenance of Effort is not required for this program. Other: 1. Applicants must have a history of experience and collaboration in the following areas: Emerging and Re-Emerging Infectious Diseases, Influenza and other diseases with pandemic potential as well as Environmental Health Issues, Chronic Diseases, Perinatal and Maternal Mortality as well as Early Childhood Health and Development. 2. Applicants will be a legal entity with approval to work with and documented proof of collaboration with Ministries of Health (MOH) and other partners within each country they are proposing Projects/activities. 3. Applicant must possess public health project implementation capacity with staff of the ground, including capacity at the local and national level. 4. Applicant must document their ability to plan and implement projects within established infrastructure thus avoiding the creation of a new vertical program. 5. Applicant must provide evidence of their capacity to carry-out educational and health programs including experience in settings of extreme poverty and working with disenfranchised individuals. 6. Applicant must provide evidence of experience working with community-level organizations in planning, implementing and evaluating evidenced-based public health programs. 7. Applicant must provide documentation of their expertise in training public health staff in disease control and prevention methods. 8. CDC seeks to expand its global capacity through NGO partnerships with organizations that have a strong international presence. Project Evaluation results in one country may be translated and implemented in other countries or settings; therefore the partnering organizations must have a worldwide network that includes resident staff present in at least 20 countries. Grant Description
The purpose of the program is to improve global health through collaboration by addressing the following public health practice goals: 1. To implement evidence-based public health programs on important human infectious diseases, with an emphasis on HIV/AIDS, Malaria, Emerging and Re-emerging Infectious Diseases, Neglected Tropical Diseases, Tuberculosis, Influenza and other diseases with pandemic potential as well as Environmental Health Issues (including Safe Water and Sanitation), Chronic Diseases, Maternal and Child Health, Public Health Preparedness, Bio-safety, Injury Control and Prevention. 2. To provide training for students, public health professionals and select staff in public health program planning, implementation and evaluation and other related issues such as data collection, data analysis, financial planning and management. 3. To incorporate the results of program evaluations and lessons learned into operational disease prevention and control programs with other nations, non-governmental agencies and academic institutions.
